Please plan to arrive 15 minutes early for your first visit in order to complete registration and medical history forms. You may also download and fill out the new patient forms and bring the completed forms at the time of your visit or fax them to 925.373.7310. This way you will be all set up in our system before you even come in the door!

If you have recent dental x-rays, you may wish to bring them with you. If you are being referred by another dentist, please bring your referral form or letter.

NOTE: If you have been recently been diagnosed with any type of heart condition, systemic infection, or plan to have any type of surgery, please call our office at 925.373.7311 so we can verify with your doctor if you need pre-treatment medications.
